Medical Release Policy
I/We agree that my dependent or I give my permission for a member of The Edge School of Performing Arts (The Edge Performing Arts Inc.), to take myself or my child to a medical/dental facility if necessary. In case of emergency, if none of the above can be contacted, we herby give our consent for emergency medical care prescribed by a duly licensed doctor of Medicine or Doctor of Dentistry. This care maybe given under whatever conditions are necessary to preserve the life, limb or well- being of my dependant. We hereby accept full responsibility for all costs of said medical care or emergency treatments. We hereby waive all claims whatsoever in connection with such medical treatments.
Waiver Release Policy
I/We Agree that if my dependent or I engage in any physical exercise, class, spectating or activity on the premises, any venue or virtual lesson where we participate as representative of The Edge School of Performing Arts (The Edge Performing Arts Inc.), we do so at our own risk. We agree that we are voluntarily participating in activity and use said facilities, premises (including parking lot) and designated The Edge Performing Arts Inc. venues. We assume all risk of injury, illness, damage, or loss to us or our property that might result, including, without limitation, any loss or theft of any personal property. The Activities have foreseeable and unforeseeable inherent risks, hazards and dangers that no amount of care, caution or expertise can eliminate, including without limitation, the potential for serious bodily injury, permanent disability, paralysis, and loss of life. These risks include: Premises: collisions with objects or barriers that are a part of the premises, dangerous, unsafe, or irregular conditions on walls, floors, the ground or other surfaces; and travel to and from the premises. Health: executing strenuous and demanding physical techniques; physical exertion; overexertion; stretching; dehydration; fatigue; cardiovascular workouts; rapid movements and stops; lack of fitness or conditioning; traumatic injury; sprains and fractures, spinal cord injuries, bacterial infections; rashes; and the transmission of communicable diseases, including viruses of all kinds, COVID-19, bacteria, parasites or other organisms. Ability: failing to act safely or within the Participant’s own ability or within designated areas. Conduct: the Participant’s conduct and the conduct of other persons including any physical altercation between participants.
I/We agree that this consent and assumption of risk statement covers each and every event or activity sponsored by The Edge Performing Arts Inc. and your affiliates and related parties an their respective directors, officers, employees, agents, instructors, insurers, shareholders, owners, operators from any and all liability and any and all claims, demands and causes of action of any kind whatsoever that we may have arising out of participation in a program or related activity, including as a result of negligence. We acknowledge that we have carefully read this Waiver and Release and full understand that it is release of liability. I/We are waiving any right that we may have to bring legal action to assert a claim against The Edge School of Performing Arts ( The Edge Performing Arts Inc.) for your negligence.
